License Certification –Specific document certifying licensure status and disciplinary history, prepared by a representative of the Division of Medical Quality Assurance or their representative that has is primary source verified. This document is generally required for applicants seeking licensure in other states and for use in court proceedings. There is a $25 fee for this service in accordance with Rule 64B-4.001.
License Verification – Information regarding the licensure status of a practitioner. This is for use by persons or organizations that do not require a document certifying this information as primary source verified or under a seal.
M.D. and D.O. Licensure - VeriDoc is a computer system that contains physician license and licnse discipline information that is provided by boards of medicine in the United States and its territories. Florida is a participating member of VeriDoc. Licensure verifications to other state medical boards can be sent through VeriDoc electronically in a minimal amount of time. Nurse Licensure Certifications - NURSYS™ (Nurse System) is a computer system that contains nurse license and license discipline information that is provided by boards of nursing in the United States and it territories. NURSYS™ received regular updates of nurses’ personal (name, address, etc.) and license information from participating boards of nursing. Florida is a participating member of NURSYS™.
