If you are unable to obtain the information or documentation you need from the applications above, you may submit a licensure public records request by contacting the Department at (850) 245-4121, completing the online form or submitting a request in writing (fees may apply).  Public records requests are not required to be in writing. You do not have to give your name or explain why you are making a request.
