Community health assessment practice is the process of systematically collecting, analyzing and using information to educate and mobilize communities, develop priorities, gather resources, and plan actions to impact the public's health. Assessment is a core function of public health practice. Assessment is the foundation for the other two core functions of public health; Policy Development and Assurance.  COMPASS improves the capacities of Florida's public health workforce to develop strategies and interventions which lead to improved population health through community partnership building, collaboration and resource sharing.

Florida MAPP Field Guide

The Field Guide is a comprehensive package for those who would implement the assessment processes. It includes descriptions of each phase, tools, tips, and other resources to make it a practical implementation guide.

A Self Evaluation Checklist (.pdf)file opens in a new window (.xlsfile opens in a new window) An evaluation tool for self-assessment to be used annually.  The self-assessment is based on the community health assessment benchmarks for Florida's county public health departments.
 
MAPP Model Training  A series of power point and pdf  training modules on the community health assessment process, using the MAPP model.

 A training program integrating the Core Functions of Public Health, the 10 Essential Public Health Services, the National Public Health Performance Standards and MAPP - Mobilizing for Action for Planning and Partnerships.

The mission of COMPASS is  to improve the capabilities of local public health systems to conduct community health assessments and apply results for improved health outcomes and public health policy formation. COMPASS builds capacity and infrastructure through:

  • Formulation of policy for systematic performance of comprehensive community health assessments.

  • Establishment of performance measures for community health assessment.

  • Development and access to Florida?s web-based data system, Community Health Assessment Resource Tool Set (CHARTS),

  • Public health workforce development through multi-modal training opportunities and technical assistance, and evidence-based evaluation and continuous improvement of the statewide initiative as well as the enhancement of local processes.
