Training Notice
The Department of Health, Bureau of Emergency Medical Services (EMS) invites the public to attend the following Grant Writing Workshops. These pertain to the state EMS grants. All will have the same presentation.
DATE AND TIME: Friday, December 12, 2008. The training will begin at 10:00 a.m. and will end by about Noon Central Standard Time.

GENERAL SUBJECT MATTER TO BE CONSIDERED: To conduct a grant-writing workshop for Emergency Medical Service (EMS) providers and other eligible organizations. The grant workshop will be conducted to provide training and information to potential grant applicants about the Florida EMS matching grant program. There is no cost or pre-registration for these workshops.

A grant training session is provided at state EMS Advisory Council meetings. Specific time and room location for a meeting can be obtained from the Constituency Meeting Schedule, at the EMS registration desk at the meeting site, or a state grant or Bureau Chief staff person before the meeting.
Additional grant training sessions will be announced at this site when scheduled.
2008-2009 County Grant Information
Florida county governments may now submit their county grant applications beginning September 29, 2008. Only the 67 Florida county governments
are eligible to apply. Use the amount listed for your county when developing your budget.

Complete the application forms in the manual, (DH Form 1684 and DH Form 1767P, Rev. June 2002) pages 3-5. Please note Item 4 in the application
requires you to include a current resolution from the Board of County Commissioners with the completed application. The resolution criteria are
described in Item 4. Note that Item 2 in the application form and the "Request for Grant Fund Distribution" form both require your original
signatures. Return a signed original and two copies of the completed application, which includes only DH Form 1684, DH Form 1767P, and the
resolution, to the following address:

The deadline by which we must receive the completed applications is no later than January 9, 2009, 5:00 PM, Eastern Standard Time. Completed applications will be processed in the sequence we receive them. If we have not received the final report from your previous county grant, it may delay the processing of the new grant application until we receive it from you.

Matching Grants
Grant Application Notice
The State of Florida, Department of Health, under section 401.113, Florida Statutes, beginning November 25, 2008 will accept applications for emergency medical services (EMS) matching grants. Your application must comply with the instructions in the June 2002 edition of the EMS Matching Grant Program Application Packet (Grant Manual), this memorandum, and other guidelines from authorized state EMS grant staff.
OUT OF COMPLIANCE: We will not consider any organizations for new matching grant funds who are out of compliance with a state EMS matching grant. Contact state EMS grant staff if you wish to confirm your organization's compliance status.
ACHIEVEMENT OF OBJECTIVES: If you receive a grant and the project's objectives are not accomplished as cited in the approved application including state revisions, the state reserves the right to require a refund of the state funds for the project. This includes grants regarding the EMS Tracking and Reporting System (EMSTARS).
DEADLINE AND NUMBER OF COPIES: We must receive your completed original and signed application form with required information pages, signed Request for Grant Fund Distribution Form, and required copies no later than 5:00 p.m. (Eastern Standard Time) February 6, 2009. We will return all applications received after the deadline. You must send the original and 10 copies for each matching grant (state pays 75 percent); but the original and only 7 copies for rural matching grants (state pays 90 percent). Send only the original and copied forms and allowable information pages (pages 3-11), NOT the entire Grant Manual, to the below address or hand-deliver to the below walk-In address. We will not accept faxed and e-mailed applications.

For your information there are two types of matching grants, general and rural, described following.
General Matching Grants. Public and private, urban and rural organizations involved in EMS are eligible for matching grant funds whereby the state pays 75 percent of approved budgets and the grantee pays 25 percent.
Rural Matching Grants. Public and private organizations involved in EMS which are based in and provide services for counties with a population of 100,000 or fewer people and a population density of less than a 100 per square mile are eligible for rural matching grants whereby the state pays 90 percent of approved budgets and the grantee pays 10 percent.
Booklet and Form. There is only one grant booklet and form for both the General and Rural matching grants. The application form differentiates between them when the applicant checks a box at the top of the form to indicate the application is for either a Rural or (General) Matching grant.
